Description

Islas et al identified factors associated with complications following transcatheter aortic valve implantation (TAVI) without balloon aortic predilation. These can help to identify a patient who may benefit from the procedure. The authors are from Hospital Clinico San Carlos in Madris.


 

Patient selection: symptomatic aortic stenosis

 

Patient evaluation: Doppler echocardiography

 

Factors associated with an unfavorable outcome following TAVI without predilation:

(1) aortic valve area < 0.4 square cm

(2) eccentric and/or irregular orifice (not central)

(3) calcification Grade 3 (severe)

(4) restricted mobility of aortic cusps Grade 3 (severely restricted)

(5) calcifications of the left ventricular outflow tract (LVOT)

(6) calcium nodules near coronary ostia or at the LVOT

(7) aortic regurgitation Grade 3 (severe)

 

where:

• Grades for calcification, mobility and regurgitation went from 0 to 3.

 

The presence of unfavorable factors was associated with complications. The absence of unfavorable factors was associated with procedural success.
