Huang et al identified risk factors for rhabdomyolysis in a patient with dengue fever. Failure to recognize rhabdomyolysis can result in complications and a worse outcome. The authors are from Kaohsiung Chang Gang Memorial Hospital and Chang Gung University in Taiwan.
Patient selection: dengue fever
Risk factors for rhabdomyolysis from multivariate analysis:
(1) hypertension, including history of hypertension
(2) myalgia
(3) acute kidney injury
Patients with myalgia often developed weakness in the extremities.
Other associations:
(1) dengue hemorrhagic fever (DHF)
(2) male gender
(3) older age (median age 65 but age range 30 to 71 years)
(4) pleural effusions
(5) elevated concentrations of TNF-alpha and IL-6
Serum CK concentration was used as the marker of rhabdomyolysis.
Complications of rhabdomyolysis in dengue fever:
(1) problems with electrolytes
(2) metabolic acidosis
(3) coagulopathy
(4) acute renal failure
