Description

Hu et al reported risk factors for incisional (superficial) surgical site infection following bowel resection for Crohn's disease. These can help to identify a patient who may benefit from interventions to reduce these complications. The authors are from the Sixth Affiliated Hospital, Sun Yat-sen University, Guangdong Provincial Key Laboratory of Colorectal and Pelvic Flood Diseases and Guangdong Institute of Gastroenterology in China.


Patient selection: bowel resection for Crohn's disease

 

Outcome: incisional surgical site infection

 

Risk factors for incisional surgical site infection on multivariate analysis:

(1) preoperative anemia (hemoglobin < 120 g/L; odds ratio 3.3)

(2) elevated percentage of neutrophils ( >75%; odds ratio 2.9)

(3) intra-operative finding of fistula (odds ratio 3.8)

 

Table 1 suggests that an elevated white blood cell count (> 10*10^9/L) is associated with SSI. A higher frequency of SSI is associated with penetrating disease (B3).

 

Suggested interventions prior to surgery:

(1) improved nutritional status

(2) reduction in inflammation
