Hsiao et al reported a number of risk factors for shock developing in a patient with urosepsis. These can help to identify a patient who may benefit from more aggressive management. The authors are from multiple institutions in Taiwan.
Patient selection: sepsis associated with a urinary tract infection
Risk factors for the development of septic shock:
(1) coronary artery disease (OR 2.5)
(2) congestive heart failure (OR 4.6)
(3) acute kidney injury (OR 3.0)
The presence of hypertension is associated with a reduced risk (OR 0.5).
The presence of risk factors identifies a patient who should be treated aggressively in order to avoid deterioration.
