Description

Holm et al identified risk factors for acute renal failure (ARF) in a severely burned patient. The presence of acute renal failure is associated with increased mortality. The authors are from Krankenhaus München-Bogenhausen and the Technical University Munich.


Patient selection: burns > 10% body surface area (BSA)

 

Risk factors for acute renal failure:

(1) greater burned body surface area (mean in study population 48% with range 13 to 95%)

(2) inhalation injury

 

Early ARF was defined as having an onset within the first 5 days.

(1) myoglobulinuria

(2) hypotension during the resuscitation phase

 

Late ARF was defined as an onset after 5 days. Late ARF was associated with:

(1) sepsis

(2) therapy with nephrotoxic antibiotics (prescribed for the sepsis)

 

Most patients who died had multi-organ failure.
