Description

Ho et al identified risk factors for poor outcome or recurrence after an episode of acute diverticulitis. These can help to identify a patient who may require more aggressive management. The authors are from Jamaica Hospital Medical Center, Memorial Sloan Ketter Cancer Center and Weill Cornell Medical College.


Patient selection: acute diverticulitis

 

91% of patients had no readmission. About 9% had 1 or more recurrence and readmission. 0.3% had 4 or more admissions.

 

Complicated diverticulitis:

(1) obstruction

(2) perforation

(3) peritonitis

(4) abscess

 

Poor outcome - any of the following during a subsequent admission (recurrence):

(1) complicated disease

(2) emergency surgery

(3) mortality

 

Risk factors for poor outcome:

(1) age <= 50 years

(2) abscess (which is included under complicated disease)

 

Elective surgery should be considered for patients:

(1) with complicated diverticulitis during first admission

(2) age <= 50

(3) have had a second admission for diverticulitis
