Description

Fux et al reported risk factors for mortality for a patient placed on venoarterial extracorporeal membrane oxygenation (ECMO) for postcardiotomy shock. This can help to identify a patient who may or may not be a good candidate for the support. The authors are from Karolinska Institute in Stockholm.


Patient selection: postcardiotomy shock

 

Outcome: 90 days mortality

 

Percent having 90-day mortality: 57%

 

Risk factors from multivariate logistic regression analysis:

(1) pre-ECMO arterial lactate in mmol/L (OR 1.22 per mmol/L)

(2) ischemic heart disease (OR 7.9)

 

where:

• The average arterial lactate for survivors was 4.0 vs 8.0 for nonsurvivors.

• Only 13% of patients with a lactate >= 10 survived.

• Only 23% of patients with ischemic heart disease survivved.
