Fujinaga et al identified risk factors for acute kidney injury (AKI) for a trauma patient in Japan. These can help to identify a patient who may benefit from more aggressive management. The authors are from Kurashiki Central Hospital in Japan.
Patient selection: trauma patient in Japan
Outcome: acute kidney injury (occurrence in 20% of trauma patients)
Risk factors for acute kidney injury:
(1) age >= 65 years (odds ratio for age 1.38 per decade)
(2) male sex (odds ratio 2.1)
(3) pre-existent chronic kidney disease (odds ratio 4)
(4) number of units of blood transfused (odds ratio 1.6 for each unit)
A patient who develops AKI is at risk for longer ICU stay and greater 28-day mortality.
