Ende et al evaluated women with postpartum hemorrhage associated with an atonic uterus. The authors are from Vanderbilt University.


Patient selection: woman with postpartum bleeding and an atonic uterus

 

Risk factors for postpartum hemorrhage with OR 2.0 or greater:

(1) history of postpartum hemorrhage of any cause (OR 2.3)

(2) placenta previa or abruption (OR 2.7)

(3) uterine rupture (OR 2.4)

(4) multiple gestation (OR 2.2)

(5) high vaginal trauma (OR 2.2)

(6) cervical trauma (OR 5.7)

 

Risk factors for postpartum hemorrhage with OR 1.5 to 1.9:

(1) macrosomia (OR 1.5)

(2) hypertension (OR 1.8)

(3) prolonged labor (OR 1.7)

(4) chorioamnionitis (OR 1.9)

(5) perineal trauma (OR 1.7)

(6) instrumental vaginal delivery (OR 1.7)

 

Risk factors for postpartum hemorrhage with OR 1.1 to 1.4:

(1) diabetes (OR 1.2)

(2) ethnicity (Hispanic OR 1.2; Asian OR 1.4)

(3) polyhydramnios (OR 1.3)

(4) induction of labor (OR 1.2)

(5) leiomyoma (OR 1.3)

(6) predelivery oxytocin exposure (OR 1.2)

(7) previous Cesarean delivery (OR 1.1)
