Description

Elmarsafi et al reported risk factors for a major lower limb amputation following reconstruction of a diabetic Charcot joint. These factors can aid in treatment planning and monitoring. The authors are from Georgetown University.


The deformity associated with the Charcot joint results in high-pressure areas that predispose to ulceration, infection and disability, with repeated need for surgical interventions.

 

Patient selection: diabetic Charcot joint (neuroarthropathy) status post surgical reconstruction.

 

Risk factors for a major lower extremity amputation:

(1) postoperative nonunion (OR 8.5)

(2) development of a new site Charcot neuroarthropathy (OR 8.2)

(3) peripheral arterial disease (OR 4.3)

(4) renal disease (OR 3.7)

(5) delayed healing after surgery (OR 2.6)

(6) postoperative osteomyelitis (OR 2.4)
