Description

Dotters-Katz et al identified risk factors for postpartum septic thrombophlebitis of pelvic veins. These can help to identify a patient who may require more aggressive management. The authors are from the University of North Carolina at Chapel Hill, University of Utah, Vanderbilt University and Duke University.


Patient selection: postpartum

 

Outcome: septic thrombophlebitis of pelvic veins

 

The patient may present with persistent febrile illness in the setting of antibiotic therapy for endometritis.

 

Risk factors from multivariate analysis:

(1) maternal age < 20 years (odds ratio 2.0)

(2) Black race (odds ratio 2.6)

(3) multiple gestation (twin, etc; odds ratio 2.1)

(4) pre-eclampsia (hypertensive disorders of pregnancy; odds ratio 2.9)

(5) Cesarean delivery (odds ratio 6.3)

(6) chorioamnionitis (odds ratio 4.8)

(7) public insurance (odds ratio 2.0)

 

Additional factors associated with septic thrombophlebitis of pelvic veins:

(1) obesity (body mass index >= 30 kg per square meter)

(2) nulliparous

(3) labor

(4) delivered preterm < 32 weeks
