Description

Dhakal et al identified factors associated with unplanned readmission following hematopoietic cell transplantation (HCT). The authors are from the Medical College of Wisconsin.


Patient selection: hematopoietic stem cell transplant, age >= 18 years

 

Outcome: 30-day readmission.

Occurrence: 12% for autologous, 24% for allogeneic

 

Reasons for readmission included:

(1) infection

(2) neutropenic fever

(3) gastrointestinal adverse effects

(4) acute renal failure

(5) dehydration with hypovolemia

(6) graft-vs-host disease (for allogeneic HCT)

 

Risk factors for readmission after autologous HCT:

(1) age < 50 years

(2) female sex

(3) disease other than myeloma, lymphoma or Hodgkin's disease

(4) Elixhauser co-morbidity score >= 20

(5) short length of stay for index hospitalization

(6) transplant at a low or medium volume center (< 187 vs >= 187 per year)

 

Risk factors for readmission after allogeneic HCT:

(1) stem cells from cord blood

(2) Elixhauser co-morbidity score >= 10

(3) transplant at a low or medium volume center (< 157 vs >= 157 per year)
