Description

Chu et al studied surgical site infections (SSIs) associated with reversal of an intestinal stoma. Stoma reversal is often considered to be a low-risk procedure but can be associated with complications in patients with significant comorbidities. The authors are from the University of Alabama at Birmingham and the Mayo Clinic.


Patient selection: reversal of an intestinal stoma

 

Complications of stoma reversal include:

(1) bowel obstruction

(2) incisional hernia

(3) anastomotic leak

(4) surgical site infection (SSI)

 

Risk factors for SSI from the multivariate analysis:

(1) smoking

(2) failure to place a subcutaneous drain

 

Other factors associated with SSIs:

(1) ASA 3 or 4 (ASA 5 would be unlikely to undergo a stoma reversal)

(2) increased length of stay

(3) other complications (sepsis, return to the operating room, etc)

(4) open closure with midline incision

 

Smoking cessation is an intervention that can reduce the risk of SSI.
