Description

Cecchini et al identified a number of factors associated with a complicated course for a patient with sickle cell disease who has been admitted to the intensive care unit (ICU). These can help to identify a patient who may require more aggressive management. The authors are from multiple hospitals in Paris.

Patient selection: adult with sickle cell disease admitted to the ICU

Risk factors for a complicated course:

(1) more aggressive acute disease within the 48 hours before ICU admission

(2) hemoglobin <= 7.8 g/dL (sustained drop)

(3) respiratory rate >= 32 breaths per minute (acute respiratory distreass)

(4) acute renal injury on ICU admission

where:

• Two of the risk factors are related to organ dysfunction/failures.
