Description

Anjewierden et al identified risk factors for Clostridium difficile infection in pediatric inpatients. These can help to identify a patient who may benefit from more aggressive management. The authors are from the Cleveland Clinic, Case Western Reserve University and the University of Kansas.

Patient selection: pediatric inpatient from 0 to 19 years

Risk factors for Clostridium difficile on meta-analysis:

(1) prior antibiotic exposure (odds ratio 2.14)

(2) use of a proton pump inhibitor (PPR, odds ratio 1.33), with gastric acid suppression

Other factors that may increase risk (not included in meta-analysis):

(1) underlying comorbidity (inflammatory bowel disease, malignancy)

(2) immunosuppression (solid organ transplant, other)

(3) prevalence of Clostridium difficile in the hospital
