Description

Nephrotoxicity may occur during vancomycin therapy. The risk may be increased if certain risk factors are present.


 

Risk factors for nephrotoxicity during vancomycin therapy:

(1) trough vancomycin levels > 10 mg/L

(2) concurrent aminoglycoside therapy

(3) concurrent amphotericin B therapy

(4) recent anesthesia

(5) male gender

(6) advanced age

(7) pancreatitis

(8) sepsis

(9) peritonitis

(10) obstructive uropathy

(11) neutropenia

(12) liver disease

 

Nephrotoxicity may occur with trough concentrations within the therapeutic range and in the absence of risk factors.
