Risk factors for a regulatory standard to creep:
(1) ambiguous wording of the original regulation
(2) failure of the credentialing organization to clarify how the rule should be interpreted
(3) inability or unwillingness to access the original regulation to verify the requirements
(4) deficient inspector training
(5) unwillingness of the site being inspected to question the inspector
(6) unconscious projection by the inspector of an inner need
(7) conscious intent by the inspector to interpret the rule in a certain way for some motive
(8) incorrect interpretation transmitted by word of mouth as fact
A site being inspected:
(1) needs to have access to the current regulations
(2) be willing to challenge an interpretation (in a professional manner)
(3) know the process of appealing a decision
(4) ask for written documentation with evidence
