Description

Hernandez-Jiminez et al reported risk factors for infection by multidrug resistant Pseudomonas aeruginosa in an immunocompromised patient. These can help to identify a patient who may require more aggressive management. The authors are from multiple institutions in Spain.

Patient selection: immunocompromised patient

Outcome: infection with multidrug resistant Pseudomonas aeruginosa

Risk factors for infection by a multidrug resistant strain:

(1) diabetes mellitus (OR 4.7)

(2) history of colonization by multidrug resistant Pseudomonas aeruginosa (OR 42.1)

(3) recent receipt of antibiotics (within past 3 months, OR 5.3)

(4) septic shock (OR 3.7)

Infection with a multidrug resistant strain of Pseudomonas aeruginosa is associated with a worse prognosis than for antibiotic-susceptible strains.

If risk factors for a multidrug resistant strain are present, then greater care should be taken to select the appropriate antibiotic to try.
