A male who has had a renal transplant may experience erectile dysfunction due to a range of risk factors.
Patient selection: male renal transplant recipient
Risk factors for erectile dysfunction:
(1) comorbidities (diabetes, hypertension, atherosclerosis, obesity, smoking, hypercholesterolemia, anemia)
(2) impact of surgery, including altered blood supply to the penis
(3) adverse drug effects
(4) psychological status (depression, anxiety)
(5) endocrine changes (hyperprolactinemia, hypogonadism, hyperparathyroidism)
(6) autonomic and/or peripheral neuropathy
(7) partner relationship
Therapies that may be effective:
(1) control of comorbid conditions
(2) correction of any endocrine imbalance
(3) phosphodieasterase-5 inhibitor (with dosing to avoid accumulation and subsequent hypotension)
(4) intracavernosal injection
(5) topical alprostadil
(6) penile prosthesis
