Physically active military personnel are at risk for lower extremity pain due to chronic exertional compartment syndrome (CECS), also known as exercise-induced compartment syndrome. Recognition of the condition allows for early therapy.
Features of CECS:
(1) pain in the lower leg associated with intense physical activity
(2) not explained by direct trauma
(3) raised intra-compartment pressure
(4) potential for ischemic necrosis of the anterior tibialis muscle (march gangrene)
(5) relief after fasciotomy
Risk factors for CECS:
(1) white race
(2) female gender
(3) older age
(4) junior enlisted rank
(5) Army service
