Description

da Silva et al identified risk factors for cardiovascular collapse in pediatric patients having an unplanned extubation. These factors can help to identify a patient who may benefit from more aggressive management. The authors are from Hospital de Servidor Publico Municipal (Sao Paulo) and the Federal University of Sao Paulo in Brazil.

The key risk factor on multivariate analysis was an age <= 6 months (odds ratio 3.4).

 

Additional factors that may present:

(1) respiratory failure associated with disease

(2) lower PaO2 to FIO2 ratio (mean 218.6 with range 132.9 to 317.9, vs mean 282.9 with mean 191.4 to 370)

(3) higher oxygenation index (mean 5.5 with range 3.3 to 12.6, vs mean 3.6 with range 2.2 to 6.1)

(4) tachypnea > 25 breaths per minute
