Risk factors:
(1) loose clothing
(2) loose jewelry
(3) long hair
(4) machinery guards removed or modified
(5) sudden release of a jam
(6) restoration of power with machine restart
(7) non-ergonomic controls
(8) slip or fall
(9) failure of a safety feature
Injuries:
(1) direct trauma (blunt, penetrating, laceration, crushing, amputation)
(2) ligature strangulation
(3) getting pulled into the machinery
(4) burns
