(1) a wall
(2) a pipe
(3) a tree
(4) stacked supplies
Risk factors for the worker:
(1) distraction (cell phone, etc)
(2) intoxication
(3) illness
(4) inexperience
Failure of safety measures:
(1) no spotter for heavy equipment
(2) failure of backup alarms
(3) failure to wear reflective clothing
(4) failure to use protective measures in a trench
(5) no inspection by a competent person
