Description

Rutherford et al listed risk factors associated with arterial disease resulting in lower extremity ischemia. The authors are from the Univerisity of Western Ontario and an Ad Hoc Committee of the Society for Vascular Surgery.

Parameters:

(1) diabetes mellitus

(2) tobacco use

(3) hypertension

(4) hyperlipidemia

(5) cardiac status

(6) carotid disease

(7) renal status

(8) pulmonary status

Parameter

Finding

Points

diabetes mellitus

none

0

adult onset, diet controlled

1

adult onset, insulin controlled

2

juvenile onset

3

tobacco use

none

0

not current, stopped within past 10 years

1

current, less than 1 pack per day

2

current, one or more packs per day

3

hypertension

none

0

easily controlled with 1 drug

1

controlled with 2 drugs

2

requires 3 or more drugs for control, or uncontrolled

3

hyperlipidemia

none

0

mild elevation, controlled with diet

1

Types II, III or IV requiring strict dietary control

2

requiring diet and drug control

3

cardiac status

asymptomatic with normal ECG

0

asymptomatic, remote MI by history (> 6 months), occult MI by ECG

1

stable angina, controlled ectomy, asymptomatic arrhythmia, drug compensated CHF

2

unstable angina, symptomatic or poorly controlled ectopy or arrhythmia, poorly compensated CHF, recent MI (within past 6 months)

3

carotid disease

no signs or symptoms, no bruit

0

asymptomatic but evidence of disease

1

transient or temporary stroke (TIA)

2

completed stroke with permanent neurological deficit

3

renal status

no known renal disease, serum creatinine < 1.5 mg/dL

0

creatinine 1.5 to 3.0 mg/dL

1

creatinine 3.0 to 6.0 mg/dL, creatinine clearance 15 to 30 mL per minute

2

creatinine > 6 mg/dL, creatinine clearance < 15 mL per minute, on dialysis, or with renal transplatn

3

pulmonary status

asymptomatic, normal chest X-ray, pulmonary function tests (PFTs) >= 80% of predicted

0

asymptomatic or mild dyspnea on exertion, mild parenchymal changes on chest X-ray, PFTs 65 to 79% of predicted

1

between 1 and 3

2

vital capacity less than 1.85 L:, FEV1 less than 1.2 L, FEV1 less than 35% of predicted, PaCO2 > 45 mm Hg, supplemental oxygen needed, pulmonary hypertension

3

total score =

= SUM(points for all 8 parameters)

Interpretation:

• minimum score: 0

• maximum score: 24

• The higher the score the greater the risk of atherosclerotic disease.
