Risk Code Matrix (RCM)

The risk code matrix is a tool for assessing the risk of an occupational task. The Hierarchy of Controls can be used to reduce the level of risk.

Parameters:

(1) severity of injury

(2) likelihood of occurrence

(3) risk code

Severity of Injury	Extremely Likely	Likely	Not Very Likely	Extremely Unlikely
fatal or permanent disability	1	1	2	3
severe injury or illness	1	1	2	3
minor injury or illness	2	2	3	3
no injury or illness	3	3	3	3

where:

• In electrical work a shock hazard >= 50V is associated with fatal or permanent disability.

Risk Code	Risk	Action
1	unacceptable	Do not perform unless risk can be reduced to level 3.
2	high	Reduce risk to level 3 before attempting.
3	low	Safe to perform. Use PPE and follow safe work practices.
