Description

The scrotum is a rare site to find a rheumatoid nodule.

Clinical features:

(1) The patient has a history of rheumatoid arthritis.

(2) The patient develops a scrotal nodule.

(3) The mass has an ill-defined margin with high signal intensity on T2-weighted scans.

(4) Biopsy of the mass shows a necrobiotic granuloma.

(5) The patient may have rheumatoid nodules elsewhere or a history of rheumatoid nodules.

Differential diagnosis:

(1) carcinoma

(2) syphilis

(3) tuberculosis

Rheumatoid nodules can rarely ulcerate, which may increase the suspicion for malignancy or syphilis.
