Description

See et al evaluated a revised National Healthcare Safety Network (NHSN) definition for mucosal barrier injury (MBI) laboratory-confirmed bloodstream infection (LCBI). The authors are from the Centers for Diseaes Control and Prevention.


Patient selection: HNSN-defined LCBI

 

Criteria - all 3 of the following:

(1) The pathogen is eligible for MBI-LCBI (see below).

(2) No other organism is isolated.

(3) The patient is eligible for MBI-LCBI (see below)

 

Criteria for the pathogen - one or both of the following:

(1) >= 1 blood culture positive for Bacteroides species, Candida species, Enterococcus species, Fusobacterium species, Peptostreptococcus species, Prevotella speices, Veillonella species or Enterobacteriaceae

(2) age <= 1 year with >= 1 of the following: core body temperature > 38°C (fever), core body temperature < 36°C (hypothermia), apnea or bradycardia

 

Criteria for the patient - one or both of the following:

(1) allogeneic hematopoietic stem cell transplant recipient within the past year and with of the following present during the same hospitalization as the positive blood culture:

(1a) Grade 3 or 4 GVHD of the gastrointestinal tract

(1b) >= 1 liter of diarrhea within 24 hours in an adult with onset <= 7 days of positive blood culture specimen collection

(1c) >= 20 mL/kg diarrhea within 24 hours for a pediatric patient with onset <= 7 days of positive blood culture specimen collection

(2) neutropenia with all of the following features:

(2a) >= 2 separate days

(2b) absolute neutrophil count or total WBC count < 500 per µL

(2c) <= 3 days of positive blood culture specimen collection
