Description

Leise et al reported several revisions to MELD (model for end-stage liver disease) which optimize the prediction of short-mortality for patients awaiting liver transplant. The authors are from the Mayo Clinic.

Patient selection: liver transplantation waiting list

Outcome: 90-day mortality

Parameters:

(1) serum total bilirubin in mg/dL

(2) serum creatinine in mg/dL

(3) INR

(4) serum sodium in mmol/L

Constraints on parameters:

(1) minimum bilirubin: 1 mg/dL

(2) maximum bilirubin for combined factor with sodium: 20 mg/dL

(3) minimum creatinine: 0.8 mg/dL

(4) maximum creatinine: 3 mg/dL

(5) minimum INR 1

(6) maximum INR: 3

(7) minimum sodium: 125 mmol/L

(8) maximum sodium: 140 mmol/L

Other adjustments: renal replacement therapy defined as creatinine of 3 mg/dL

refitted MELD score =

= (4.258 * LN(bilirubin)) + (6.792 * LN (creatinine)) + (8.29 * LN(INR)) + (0.652 * (140 - (sodium))) - (0.194 * (140 - (sodium)) * (bilirubin)) + 6.327
