A Working Group of the Acute Dialysis Quality Initiative (ADQI) and International Ascites Club (IAC) has proposed a classification of renal dysfunction in a patient with cirrhosis. The authors are from multiple institutions from around the world.
Patient selection: cirrhosis
Criteria for acute kidney injury (includes Hepatorenal Syndrome Type 1) - one of the following:
(1) rise in serum creatinine concentration >= 50%
(2) rise in serum creatinine concentration >= 26.5 µmol/L in less than 48 hours
Criteria for chronic kidney disease (includes Hepatorenal Syndrome Type 2):
(1) eGFR by the MDRD-6 formula < 60 mL/min for more than 3 months (MDRD-6 uses 6 variables: age, sex, serum creatinine, BUN, albumin and race)
Criteria for acute-on-chronic kidney disease - both of the following:
(1) one of the criteria for acute kidney injury
(2) the criteria for chronic kidney disease
