Aresta et al reported findings that may be present in a patient with Type 2 diabetes and hidden hypercortisolism. The authors are from multiple institutions in Italy and the United States.
Patient selection: type 2 diabetes
Red flags for possible hidden hypercortisolism:
(1) arterial hypertension (OR 1.9)
(2) need for insulin therapy (OR 2.3)
(3) advanced Type 2 diabetes (OR 3.4; more advanced than expected for age)
where:
• Advanced Type 2 diabetes = (a) microvascular and/or macrovascular complications; (b) insulin treatment plus hypertension, or (c) hypertension requiring 2 or more drugs for control.
• The presence of advanced Type 2 diabetes would supersede the other 2 if present.
