Description

Yeo et al used the ratio of serum D-arabinitol to serum creatinine to evaluate patients with Candida fungemia. This can help identify patients who are at high risk for short term mortality and who may benefit from more aggressive management. The authors are from the VA Connecticut Healthcare System and Yale University.


 

Parameters:

(1) serum D-arabinitol in micromoles per liter (µmol/L or µM)

(2) serum creatinine in mg/dL

 

ratio of D-arabinitol to creatinine =

= (serum D-arabinitol in µmol/L) / (serum creatinine in mg/dL)

 

where:

• Units are in µM per mg per dL. This works out to µM • dL / mg, which can be further reduced by taking out the volume in L.

• The molecular weight of D-arabinitol is 152.1.

 

Interpretation:

• A ratio >= 3.9 µM per mg per dL 3 or more days after the presumed onset of the Candidemia was associated with increased mortality within the next 30 days (67% if present vs 17% if absent).
