C-peptide immunoreactivity (CPR) is a marker of beta-cell function. The postprandial ratio of CPR to glucose (CPR index) is predictive of future insulin therapy.
Patient selection: type 2 diabetic
Specimen collection:
(1) after overnight fast
(2) 2 hours after a breakfast eaten after an overnight fast
Parameters:
(1) C-peptide immunoreactivity (CPR) in ng/mL
(2) glucose in mg/dL
ratio =
= (CPR) / (glucose) * 100
Interpretation:
• The ratio was 2.7 +/- 1.6 (1.1 to 4.3) for patients who did not require insulin.
• The ratio was 1.5 +/- 0.9 (0.6 to 2.4) for patients who did require insulin.
• The adjusted OR for the ratio was 0.4.
Performance:
• The area under the ROC curve is 0.78.
