Description

Aglietti et al described radiographic stages of spontaneous osteonecrosis involving a femoral condyle at the knee. This is a modification of the classification by Koshino. The authors are from the Hospital for Special Surgery in New York and the University of Florence.


Radiographic Findings in the Affected Knee

Stage

normal

I

flattening of the affected weight-bearing portion of the femoral condyle

II

radiolucent area surrounded proximally and distally by some sclerosis without frank sequestration

III

radiolucent area surrounded by a sclerotic halo

collapse of subchondral bone forming a calcified plate (sequestrum)

IV

secondary degenerative change (joint space narrowing, subchondral sclerosis, osteophyte formation, erosions) in both femur and tibia
