Description

Exposure of the heart to radiation can result in a range of damage to the heart which can be severe.


Radiation-induced effects:

(1) coronary arteriosclerosis

(2) microvascular dysfunction

(3) conduction disturbances (RBBB, AV block, etc)

(4) valvular disease with stenosis or insufficiency

(5) heart failure with systolic and/or diastolic dysfunction

(6) restrictive cardiomyopathy

(7) myocarditis

(8) pericardial disease (pericarditis, pericardial effusion, constrictive pericarditis)

(9) myocardial fibrosis

 

Risk factors for cardiotoxicity (Jaworski et al):

(1) total radiation dose > 30 Gy

(2) high dose fraction (> 2 Gy per day)

(3) volume of heart irradiated

(4) presence of tumor next to the heart

(5) younger age at exposure

(6) number of years since the exposure

(7) type of radiation source (cobalt)

(8) cardiotoxic chemotherapy

(9) risk factors for atherosclerosis (diabetes, smoking, etc)

(10) relative field weighting
