(1) pulse in beats per minute
(2) respiratory rate in breaths per minute
(3) systolic blood pressure in mm Hg
(4) fever in °C
(5) state of consciousness

Interpretation:
• minimum qMEWS: 0
• maximum qMEWS: 5
• The higher the score the greater the physiologic disturbance,
• A qMEWS >= 2 indicates need for an increased level of care.
