Description

Audureau et al reported the PURE-4 scale to screen for psoriatic arthritis in patients with psoriasis. This can help to identify a patient who should be referred to a rheumatologist for evaluation. The authors are from multiple institutions in France.


Patient selection: psoriasis

 

Parameters:

(1) ever had a globally swollen and painful finger or toe (dactylitis)

(2) ever had heel pain when first standing up in the morning (inflammatory heel pain)

(3) ever had bilateral buttock pain

(4) ever had a swollen and painful peripheral joint before the age of 50 years

Interpretation:

• minimum score: 0

• maximum score: 4

• A score >= 1 identifies a patient who should be screened for psoriatic arthritis.

 

Performance:

• At a cutoff >=1, the sensitivity was 86% and specificity 84% with area under the ROC curve 0.88.
