Description

A patient with AIDS may develop a mixed pulmonary infection caused by a Trichomonas species and Pneumocystis jirovecii (formerly P. carinii). The presence of pulmonary trichomoniasis often goes undetected because it is not expected.

Findings:

(1) acute respiratory distress syndrome (ARDS) in a patient with AIDS

(2) ground-glass opacities on chest X-ray

(3) presence of Pneumocystis

(4) presence of Trichomonads

Testing is needed to distinguish between the Trichomonas species that could be present:

(1) T. tenax (from the oropharynx)

(2) T. vaginalis (from the genital tract)

(3) T. hominis (from the gastrointestinal tract)
