Orbital myositis may be associated with altered function in one or more ocular muscles. One consequence may mimic Duane Retraction Syndrome (DRS).
Mechanism: involvement of the lateral rectus muscle
Clinical features:
(1) gaze-evoked amaurosis
(2) ocular pain
(3) diploplia
(4) restricted adduction
After therapy was started there was narrowing of the palpebral fissure on attempted adduction (illustrated in Figure 1 of the article).
The diagnosis requires exclusion of other causes of pseudo-DRS such as orbital trauma.
