Description

Proton pump inhibitor responsive esophageal eosinophilia is in the differential diagnosis of eosinophilic esophagitis.


Features of proton pump inhibitor responsive esophageal eosinophilia:

(1) esophageal eosinophilia with >= 15 eosinophils per high power field (hpf)

(2) clinical and endoscopic features of eosinophilic esophagitis

(3) complete remission with proton pump inhibitor therapy (including reduction in esophageal eosinophils)

 

The chief complaint is intermittent dysphagia with or without food impaction.

 

Endoscopic features:

(1) loss of vascular markings

(2) rings

(3) white exudates

(4) longitudinal furrows

(5) narrowing of the esophagus with or without stricture

 

Differential diagnosis:

(1) GERD related esophageal eosinophilia, with symptoms of acid reflux and evidence of acid in the esophagus on pH monitoring
