Antibiotic prophylaxis is indicated before biliary tract surgery only in patients at high risk for infection.
High risk patients for infection:
(1) age > 70 years of age
(2) acute cholecystitis
(3) non-functioning gallbladder
(4) obstructive jaundice
(5) common duct stones
Likely pathogens:
(1) enteric Gram-negative bacteria
(2) enterococci
(3) clostridia
Recommendation for prophylaxis: cefazolin 1-2 grams IV
