San Roman et al developed a prognostic score for a patient with left sided infective endocarditis. This can help to identify a patient who may benefit from more aggressive management. The authors are from multiple hospitals in Spain and Argentina.
Patient selection: left-sided endocarditis
Outcome: death or urgent surgery (performed prior to adequate preoperative antibiotic therapy)
Parameters identified during the first 72 hours after admission:
(1) heart failure
(2) organism
(3) perivalvular complications (abscess, pseudoaneurysm, fistula)
