Description

Vincent et al identified clinical features which can help stratify patients with small cell lung cancer into prognostic groups. This can help identify patients who may benefit from more aggressive management. The authors are from Royal Marsden Hospital in England.


 

Parameters:

(1) ECOG performance scale

(2) serum albumin

(3) serum ALT

(4) liver scan

 

Criteria for good prognostic group - all of the following:

(1) ECOG performance scale 0 or 1

(2) serum ALT normal

(3) serum albumin >= 3.6 g/dL

(4) normal liver scan

 

Criteria for intermediate prognosis - 1 or 2 of the following:

(1) ECOG performance scale 2

(2) elevated serum ALT

(3) serum albumin 3.0 to 3.5 g/dL

(4) abnormal liver scan indicative of metastatic disease

 

Criteria for poor prognosis: anyone who is not classified as having a good or intermediate prognosis

 

Features that tend to be found in a patient with poor prognosis (not covered in the previous groups):

(1) serum albumin < 3.0 g/dL

(2) ECOG performance scale 3 or 4
