Description

Zhang et al identified prognostic factors for an HIV-negative patient who develops Pneumocystis jirovecci pneumonia. These can help to identify a patient who may benefit from more aggressive management. The authors are from  Second Military Medical Universtiy in Shanghai and Nanjing University.


Patient selection: HIV-negative with Pneumocystis jirovecii pneumonia

 

Risk factors in patient group:

(1) immunocompromising condition

(1a) hematologic malignancy

(1b) long-term systemic corticosteroids

(2) failure to receive prophylaxis

 

Risk factors for severe disease:

(1) age > 43 years

(2) dyspnea

(3) serum LDH > 404 U/L

 

Risk factors for mortality:

(1) serum LDH > 424 U/L

(2) PaO2 < 60 mm Hg

(3) absolute monocyte count < 0.2 * 10^9/L

(4) absolute lymphocyte count < 0.3 * 10^9/L
