Description

Yang et al evaluated factors impacting survival after surgery for vertebral metastases. These can help to identify patients with the worst prognosis. The authors are from Korea Cancer Center Hospital and the Korea Institute of Radiological and Medical Science in Seoul.

Patient selection: metastatic cancer to vertebral spine, with exclusion of hematologic malignancies (myeloma, lymphoma)

Outcome: postoperative survival in months

General indications for surgery:

(1) spinal instability

(2) progressive symptomatic deformity

(3) neurologic deficits

(4) intractable pain

Factors associated with poor postoperative survival in multivariate analysis:

(1) preoperative systemic condition (Tomita score >= 6)

(2) postoperative nonambulatory status

where:

• The Tomita score includes a parameter for rapid tumor growth).
