Sites of origin of a pancreatoduodenal carcinoma:
(1) ampulla of Vater
(2) distal common bile duct
(3) head of the pancreas
(4) extra-ampullary duodenum
Prognostic factors from multivariate analysis:
(1) venous invasion
(2) perineural infiltration
(3) positive surgical margin
(4) grade of tumor differentiation
Additional factors:
(5) early diagnosis (early onset of jaundice with ampullary carcinoma)
(6) lymph node metastases
The prognosis of pancreatoduodenal carcinoma tends to be poor, especially for pancreatic adenocarcinoma.
