Wakamatsu et al identified a number of prognostic factors for a patient with miliary tuberculosis. These can help to identify a patient who may benefit from more aggressive management. The authors are from Omuta National Hospital, University of Chikushi, and Fukuoka National Hospital in Japan.
Patient selection: miliary tuberculosis
Outcome: 3-month mortality rate
Risk factors for short-term mortality:
(1) age > 78 years (odds ratio 15.5)
(2) occurrence of ARDS during the disease course (odds ratio 12)
(3) disturbance of consciousness (odds ratio 81.5)
(4) high serum urea concentration (> 13 mmol/L, odds ratio 5.7)
where:
• These reflect organ failures for lung, brain and kidneys, respectively.
