Description

Villar-del-Moral et al reported prognostic factors for a patient with parathyroid carcinoma. This can help to identify a patient who may benefit from more aggressive management. The authors are from multiple institutions in Spain.


Patient selection: parathyroid carcinoma

 

Factors associated with recurrence:

(1) intraoperative tumor rupture (hazard ratio 6.2)

(2) mitotic activity in tumor cells (hazard ratio 4.8)

(3) Schulte stage III (T4 N0M0 or TanyN1M0; hazard ratio 5.2)

 

where:

Stage IV would also impact recurrence but disease-specific survival might be more important.

 

Factors associated with poor disease-specific survival:

(1) intraoperative tumor rupture (hazard ratio 58.7)

(2) distant recurrence (hazard ratio 38.7)

 

Prognosis is greatly influence by the surgeon's performance (and probably adequacy of the original excision).
