Description

Vermeulen et al identified prognostic factors for a patient who has had a benign esophageal perforation. These can help to identify a patient who may benefit from more aggressive management. The authors are members of the Benign Esophageal Perforation Collaborative Group and are from multiple institutions in Europe and North America.

Patient selection - benign esophageal perforation due to:

(1) iatrogenic perforation

(2) Boerhaave's syndrome

Predictors of a better clinical outcome:

(1) early diagnosis (<= 24 hours) and treatment

(2) iatrogenic perforation (30-day survival 94% for iatrogenic vs 88% for Boerhaave's)

(3) age < 70 years
