Description

Tang et al identified prognostic factors for a patient with microscopic polyangiitis and diffuse alveolar hemorrhage. These can help to identify a patient who may require more aggressive management. The authors are from the First Affiliated Hospital of Chongqing Medical University in China.


Patient selection: microscopic polyangiitis (mPA) with diffuse alveolar hemorrhage (DAH)

 

Median survival time: 46 months

 

Prognostic factors from multivariate analysis:

(1) age > 65 years of age (HR 4.3)

(2) serum creatinine > 500 µmol/L (HR 2.0, indicative of renal involvement)

(3) PaO2/FIO2 ratio < 300 mm Hg (HR 4.1)

(4) lung involvement area >= 50% (HR 2.9)

 

Serum LDH > 500 IU/L was associated with a worse prognosis in the univariate analysis.
