Description

Pfannschmidt et al identified prognostic factors for patients undergoing resection of pulmonary metastases from a renal cell carcinoma. These can help to identify a patient who may benefit from more aggressive management. The authors are from Thoraxklinik-Heidelberg in Germany.

Patient selection: resection of metastatic renal cell carcinoma to the lungs

Poor prognostic factors following complete resection:

(1) number of pulmonary metastases > 7

(2) presence of pulmonary or mediastinal lymph node metastases

(3) duration of the disease-free interval between nephrectomy and pulmonary resection <= 23 months

Number of Poor Prognostic Factors

Median Overall Survival from time of surgery

0

8 years

1

3.5 years

2

2 years

3

1 year

Additional factors associated with a worse prognosis:

(1) incomplete resection of metastases

(2) synchronous metastases.

Performance of metastasectomy for recurrent disease was found to be beneficial.
